IT IS DONE ON A 8.5 x 11 SHEET OF PAPER. I ACQUIRED A NICE COLLECTION OF THESE QUESTIONNAIRES (MOST ARE FROM THE MID 90's) AND WILL BE LISTING MANY OF THEM FOR SALE THIS WEEK ON EBAY.
